    NOMINATION & APPLICATION FORM FOR PMHA BOARD POSITION
    This form is for members wishing to apply for a PMHA board position during the elections process. As per 7.1 of the PMHA by law the following PMHA members in good standing may complete and submit this form: An individual eighteen years of age or older; A person who has not been found incapable of managing property under the Substitute Decisions Act, 1992 or the Mental Health Act or similar current legislation; A person who has not been found incapable by any court in Canada or elsewhere; Not bankrupt

    This can be completed by a member of the PMHA to bring forward a motion to the monthly Board meeting, or to the AGM for consideration. All sections of the document must be completed for consideration. This form can be hand written or typed - therefore both the pdf and Word versions are included for your convenience. Please deliver the completed form to a Board member and the form must be received by the PMHA Secretary at least 72 hours before the next monthly Board meeting, or by the posted deadline in advance of the AGM meeting (which is usually 14 days).
