B E F O R E COMPLETING THIS FORM PLEASE OBTAIN AND SUBMIT
1. An original VULNERABLE SECTOR Police Check, and
2. Complete the Respect in Sport Activity Leader course (links and instructions found on the PMHA website on the Volunteer Info tab on the "What you need to know" document).
Once you have received your vulnerable sector police check, deliver it to PMHA and complete and submit this form in order to become an "On Ice Volunteer". PMHA will inform you & the coach when you are clear to be on the ice.
