On-ice Student Volunteer Opportunities - Registration Form
If you are a Midget or Bantam aged player in the PMHA, and are interested in volunteering with some of our PMHA programming at our youngest age groups, that could count towards your school volunteer hours, or just because you are interested in volunteering, please submit this form. The PMHA will start a list of those interested and will be in touch with potential opportunities including activity, time and date. The PMHA will be responsible for signing off on Volunteer hours to the schools and may also be able to provide letters of reference upon request. Deadline - This form will close on Sept 28 at 5:00pm.

All hours must be signed off by the designated PMHA Board member using the proper form required by each school. This will be coordinated by the PMHA President.

Hours will only be authorized if the volunteer performed the duties properly and responsibly. This is required active assistance to help make our PMHA programs run efficiently and purposefully and to help in teaching the game of hockey to our youngest and newest Panther players. Goofing off, disrespect or inconsiderate behaviour will result in ejection (such as shooting slapshots around the ice, or not participating and just standing in the corner).

Volunteers must be present at the designated time for the hours to count towards their community service submission.

Volunteers who are not following the requests/guidance/on-ice rules and instructions of the adult(s) supervising the activity (ie: the Head Coach) will be asked to leave, and will be disqualified from further hours.

All volunteers must be at least 2 years older than the division they assist with.

All volunteers must understand and follow the OMHA Rules regarding equipment usage on ice - there are no exceptions.  (Under 14 must wear full equipment. 14 and older can opt to wear only helmet and gloves although we do not recommend this in all instances). The age is determined on the actual date of volunteering.

All communication will be done through the parent's email address - the PMHA will not be communicating directly to players via electronic means.
