PMHA Confirmation of Double Vaccination 

 In accordance of Ontario Minor Hockey Association’s (OMHA) Covid 19 Vaccine Policy,  Pelham Minor Hockey Association (PMHA) is required to verify proof of double vaccination.  As per the policy OMHA is mandating that all players aged 12 years and older, coaches,  officials, and volunteers are fully vaccinated by Oct. 31, 2021. PMHA Confirmation of Double Vaccination 

 In accordance of Ontario Minor Hockey Association’s (OMHA) Covid 19 Vaccine Policy,  Pelham Minor Hockey Association (PMHA) is required to verify proof of double vaccination.  As per the policy OMHA is mandating that all players aged 12 years and older, coaches,  officials, and volunteers are fully vaccinated by Oct. 31, 2021. If not fully vaccinated,  the individual will be suspended from any further participation in minor hockey activities, as  of Nov. 1, until they become fully vaccinated. 

Any affected individual may apply for an accomodation exemption to this mandate, with  medical documentation, directly with OMHA. If approved, the accommodation exemption  can then be provided to PMHA. Please note PMHA can not provide the exemption and all  enquiries must be directed to OMHA. 

Starting Saturday, September 25, 2021, the PMHA Executive Directors of each division will  be at the arena asking to view only your proof of vaccination. This process will be ongoing  so please ensure that you have your vaccination record with you until it has been viewed  and noted. Please note that no vaccination records will be collected, recorded, or copied. Please review the (OMHA) Covid 19 Vaccine Policy for further information.
